9. Fotocopy KK Calon Istri - 2 lbr

10. Waktu Pelaksanaan Akad Nikah (Hari / Tanggal / Jam)

* Catatan  Bagi Duda Talak / Cerai

11. Akte Cerai Asli + Lampiran - 1 lbr

12. Fotocopy Akte Cerai

* Bagi Duda Mati

13. Surat Nikah yang asli - 1 lbr
